When teams build automations without engineering involvement, what actually breaks at scale?

Data quality was an issue, but it was manageable because we could document standards and train people on it. The real problem was visibility. When 30 people are building workflows in parallel, nobody knows if workflow A in marketing is using the same customer data source as workflow B in operations.
